Our practice closed March 2014.
We began the practice in early 2004. Now Dr. Van Dyke works outside of the area, but we are available to provide copies of your medical records for you primary care doctor or other providers.
For all those patients who have entrusted us with your care over the years, it has been my privilege to be your physician.
Medical Records request:
You can request a copy of your medical records. We do not charge to send a copy of your most recent note to another doctor's office. This note includes a summary of all your previous skin cancers (if any) as well as a list of medications and problems treated. This is usually all that is needed for continuation of care. You can make this request through our email address, BUT be aware that information sent this is way is NOT secure, so we do not recommend sending any sensitive health information.

There is a charge of $10 for the first 30 pages, and you can mail this in with the printed request OR pay onlinePAY BILL ONLINE by entering the patient name and clicking on the "Pay Now" button.
